Long-Term Care + Life Insurance

This hybrid Long-Term Care + Life Insurance benefit blends coverage for in-home and other long-term medical care needs for you, as well as life insurance for you and your loved ones.

  • Select coverage for yourself or your eligible spouse.
    • Employees under the age of 70 can elect up to $150,000 without answering any health questions during this special Open Enrollment.
    • Employees over the age of 70 are eligible to enroll but are required to go through a simplified underwriting process.
  • You can use the value of your death benefit to cover eligible long-term care services for you and your covered spouse (following a 90-day elimination period). You can also draw on your life insurance policy each month, capped at 4%, to cover qualifying long-term care expenses.
  • If you pass away, a lump-sum cash benefit is paid directly to your beneficiary.
  • Your death benefit can be passed to your beneficiary free from state or federal estate taxes.

After electing coverage in ADP, you may enroll by visiting allstate.benefitselect.com. At the Employee Login screen, enter your full Social Security number (SSN) and your Personal Identification Number (PIN). Your PIN is a combination of the last four digits of your SSN and the two-digit year of your birth. Example: If the last four digits of your SSN are 3214 and you were born on September 21, 1968, your PIN would be 321468.

Who You Can Cover

Yourself Choose $10,000, $25,000, $50,000, $75,000, $100,000 or $150,000 of coverage.
Your Spouse Choose $10,000, $25,000, $50,000 or $75,000 of coverage for a working spouse or $10,000 for a non-working spouse.

You pay for this benefit through payroll deductions, based on who is enrolled (yourself, your spouse), tobacco status and the level of benefit you elect. Rates are available when you enroll through the Allstate Enrollment Center.

Hospital Insurance

If you or a covered family member is admitted to the hospital for a covered accident, illness or childbirth, Hospital Insurance through Unum pays a benefit to help cover out-of-pocket expenses.

  • The benefit is paid directly to you, not to the hospital or provider.
  • Hospital Insurance is compatible with all Personify Health medical plans and Health Savings Accounts (HSAs).
  • You may take the coverage with you if you leave the Company or retire.
  • Each covered family member can earn $50 for completing certain Be Well preventive screenings.

Critical Illness Insurance

Critical Illness Insurance through Unum pays a benefit directly to you. You can use that money to cover any out-of-pocket expenses you choose. Covered illnesses include:

  • Heart attack
  • Stroke
  • Major organ failure
  • End-stage kidney failure
  • Coronary artery disease
  • Certain cancers
  • Certain progressive diseases like Amyotrophic Lateral Sclerosis (ALS), Dementia, Alzheimer’s disease, and Multiple Sclerosis (MS)

Who You Can Cover

Yourself Choose $10,000, $20,000 or $30,000 of coverage with no medical underwriting to qualify if you apply during Open Enrollment.
Your Spouse Spouses can get up to 100% of the employee coverage amount. You must enroll for coverage yourself to enroll a spouse.
Your Child(ren) Children from live birth to age 26 are automatically covered at no extra cost. Their coverage amount is 100% of yours. They are covered for all the same illnesses, plus these specific childhood conditions: cerebral palsy, cleft lip or palate, cystic fibrosis, Down syndrome, and spina bifida. The diagnosis must occur after the child’s coverage effective date.

You pay for this benefit through payroll deductions, based on who is enrolled (yourself, your spouse, your child(ren)) and the level of benefit you elect. Every year, each covered family member can also receive a payment for getting a covered Be Well Benefit screening test.

Accident Insurance

Accident Insurance through Unum pays a set benefit amount directly to you for covered injuries that result from accidents that occur on or off the job. You can use that money to cover any out-of-pocket expenses you choose. And, because it’s portable, you can take your Accident Insurance coverage with you if you leave the Company or retire.
