Personify Health provides Basic Life and AD&D Insurance, Short-Term and Long-Term Disability, and certain types of leave. You can also purchase additional Voluntary Term Life Insurance and AD&D coverage for added peace of mind.
Life and AD&D Insurance
It’s important to make sure your family will be taken care of in the event of your death. The Company pays for:
- Employee Life Insurance up to one times your annual salary, not to exceed $500,000
- Employee Accidental Death and Dismemberment (AD&D) Insurance up to one times your annual salary, not to exceed $500,000
Basic Life and AD&D Insurance are available effective upon your date of hire.
Short-Term Disability (STD) & Long-Term Disability (LTD)
Company-paid disability coverage provides income replacement if you need to be out of work due to illness or injury. You are eligible for STD and LTD upon your date of hire.
- STD provides 60% per week gross income (up to $3,400) for the first 26 weeks following illness or injury. There is a seven-day waiting period before benefits begin.
- LTD provides income replacement up to 60% of your annual salary (up to $10,000 per month) up to age 65. It picks up where STD ends if you need to be out of work for an extended period.
Note that you do not need to enroll in Basic Life/AD&D, STD or LTD. You will need to make an active election when you enroll if you wish to purchase additional Voluntary Term Life or AD&D coverage for yourself, your spouse or your children.
Important note about disability insurance for employees who work in statutory disability states: Unum will offset your company-provided STD benefits by any other sources of income including state disability benefits.
Family and Medical Leave Act (FMLA)
All employees of Personify Health who have been employed for at least one year and completed at least 1,250 hours of service during the 12-month period immediately preceding the commencement of leave, are eligible for up to 12 weeks of leave (or 26 weeks, as applicable) under the FMLA.
Note: In most cases, leaves will run concurrent with FMLA and any applicable state leave entitlements.

Leave of Absence
Personify Health offers the following types of leave.

| Parental (non-birthing parent) | Full-time employees scheduled for at least 30 hours weekly after 90 day waiting period; must be taken within the first year of birth | 100% of weekly earnings up to 8 weeks for bonding, offset by any applicable state family leave payments |
| Parental (adoption/foster of child up to age 16) | Full-time employees scheduled for at least 30 hours weekly after 90 day waiting period; must be taken within the first year of placement | 100% of weekly earnings up to 8 weeks, offset by any applicable state family leave payments |
| Short-Term Disability (employee serious health condition) | Full-time employees scheduled for at least 30 hours weekly | 60% of weekly earnings not to exceed $3,400 per week for a period of up to 26 weeks* |
