From medical, dental and vision to additional insurance and voluntary benefits, it’s important to consider all your options and make your 2025 benefits elections on time.
Eligibility
All regular full-time employees are eligible to enroll in Personify Health benefits. You may also enroll your eligible dependents, including:
- Your legal spouse or domestic partner
- Your children under age 26 — biological, step or adopted child(ren)
If you are covering a spouse or domestic partner, you will receive instructions following enrollment to confirm their eligibility. Please provide all necessary documentation by the requested deadline. Failure to do so will result in termination of coverage for your covered spouse/domestic partner.

Dependent Eligibility
As part of our fiduciary responsibility, we will be conducting a dependent eligibility audit in early 2025. You will need to provide the requested proof of dependent eligibility for any covered dependents. You will receive information later this year describing information and documentation you will need to provide and the steps you will need to take to confirm your dependents’ eligibility. You will need to submit the requested documents in a timely manner as indicated in the materials you receive, or your dependents will be dropped from coverage.
Making Changes During the Year
Once you enroll, you may only make changes to your 2025 benefit elections during the year if you experience a recognized Qualified Event such as marriage, divorce or the birth or adoption of a child. You typically have 31 days following a Qualified Event to request a change.
