From medical, dental and vision to additional insurance and voluntary benefits, it’s important to consider all your options and make your 2025 benefits elections on time.

Active Eligibility

All regular full-time employees are eligible to enroll in Personify Health benefits. You must enroll in plans within 30 days of your eligibility date (new hire or status change). All elections except commuter benefits and retirement elections become effective on your eligibility date. Commuter benefits and retirement elections commence on the first of the month following your eligibility date. You may also enroll your eligible dependents, including:

  • Your legal spouse or domestic partner
  • Your children under age 26 — biological, step or adopted child(ren)

If you are covering a spouse or domestic partner, you will receive instructions following enrollment to confirm their eligibility. Please provide all necessary documentation by the requested deadline. Failure to do so will result in termination of coverage for your covered spouse/domestic partner.

Dependent Eligibility

Employees with enrolled dependents are subject to an annual dependent verification audit, generally in January each year.

If you enroll a domestic partner in Personify Health benefits, you must complete the domestic partner attestation. Under federal tax law, the value of your domestic partner’s coverage must be included in your gross income and be subject to federal income tax withholding and employment taxes and will be reported on your Form W-2. This includes any portion of the premiums that Personify Health pays for your domestic partner’s health coverage. Applicable state taxes may apply. The Benefits team will send the domestic partner attestation document to impacted employees.

Making Changes During the Year

Once you enroll, you may only make changes to your 2025 benefit elections during the year if you experience a recognized Qualifying Life Event (QLE). Common QLEs and required supporting documentation are listed below. All QLEs except for birth of a child and adoption of a child are processed with an effective date of the first of the month following documentation submission with the requirement to submit documentation within 30 days of the event date. Changes to benefits due to birth or adoption are effective retroactive to the date of birth or adoption. All documentation must be submitted within 30 days of the event date.
