Your Personify Health Benefits
At Personify Health, we are proud to offer a comprehensive, competitive benefits package that supports all aspects of your wellbeing — physical, financial and emotional.
You can choose from four medical plan options and a variety of new voluntary benefits, browse our Employee Savings site for exclusive discounts on travel, retail, pet insurance, and so much more.
To get the right coverage for next year, keep these tips in mind:

Medical & Pharmacy
- Choose from four medical plan options:
- HealthChoice and HealthChoice Plus, two new Reference-based Pricing (RBP) plans that allow you to see any provider you choose and offer the lowest per-paycheck contributions (only available in select geographical locations — about two-thirds of all Personify Health employees will be eligible).
- The High Deductible Health Plan (HDHP) that offers a Health Savings Account (HSA) to which both you and Personify Health contribute to help cover your out-of-pocket costs.
- The Traditional Plan, a Preferred Provider Organization (PPO) plan.
- Note key plan design features and costs — including deductibles, per-paycheck costs and availability based on location — to ensure you pick the right coverage at the right price for next year.
- Premium discounts are available based on completion of various activities in the Personify platform, which helps lower your 2025 costs for coverage.
- Pharmacy benefits are tied to the medical plan you choose, with different deductibles and costs for retail, mail order, and specialty medications. The amount you pay may also vary based on the formulary status of your prescriptions (e.g., generics, preferred brands).
- Express Scripts, via RxBenefits, is the pharmacy benefits manager (PBM) for all four medical plans.
- Our own third-party administrator (TPA), which is legacy HealthComp, will now handle our claims processing. Any Member Service Representatives handling employee medical data have signed confidentiality agreements.
- Cigna PPO will be the network for the two non-RBP plans (Traditional PPO and HDHP).
Dental
Comprehensive dental coverage is available through a Preferred Provider Organization (DPPO) through Cigna’s Preferred Provider Organization (DPPO) Total Plus network. Choose between the Core and Buy-Up options. The Core option covers basic, preventive and major care. The Buy-Up option includes orthodontia.
Vision
Our vision plan will be offered through EyeMed’s Insight Network. You can see any licensed vision provider but will have a higher level of coverage using in-network providers. Annual preventative eye exams are covered at a $0 copay with EyeMed PLUS providers. Discounts and allowances for frames and contact lenses are also available.
Health Savings Account (HSA)
If you participate in the HDHP or HealthChoice Plus plan, you are also eligible to participate in an HSA. It allows you to set aside pre-tax dollars, up to annual IRS limits, to pay for eligible health care expenses, including your deductible.
In addition, Personify Health makes an annual contribution to your HSA, depending on the level of coverage you elect ($500 for Employee only or $1,000 for Family).
Life, AD&D and Disability
- Personify Health provides Basic Life and Accidental Death and Dismemberment (AD&D) Insurance, as well as Short-Term Disability (STD) and Long-Term Disability (LTD) coverage, through Unum. You do not need to enroll in these benefits.
- You can also choose to purchase additional Voluntary Term Life Insurance and AD&D coverage for yourself and eligible dependents.
- Be sure to note the benefit periods for STD and LTD, as well as adjusted coverage for Life Insurance and STD.

- Exclusive Employee Savings site, where you can browse exclusive deals, discounted home, auto, and pet insurance, and more.
- Long-Term Care + Life Insurance that helps cover in-home and other long-term medical care needs, in addition to life insurance.
- Hospital Insurance that pays a cash benefit to you if you are admitted to the hospital.
- Critical Illness Insurance that pays a lump-sum benefit to you if you are diagnosed with a covered illness or condition.
- Accident Insurance that pays a benefit directly to you or a covered family member experiences a covered incident.
- Identity Theft Protection to help protect your personal and financial information online.
- Legal Plan that offers access to attorney and resources to support a range of legal matters.
- Care.com provides access the world’s largest network of background-checked caregivers for short- and long-term care, as well as 24/7 access to one-on-one expert support for life’s daily distractions and major decisions.
- Added pre-tax savings for eligible health care and dependent care expenses through Flexible Spending Accounts (FSAs).
- Employee Relief Fund to help employees experiencing hardship. You can request a grant if you experience a hardship and make tax-free contributions to help your fellow employees during trying times.
- Technology stipend, which is only provided to those remote employees in California, Massachusetts, and Illinois to align with requirements.
- Commuter benefits through Benefit Extras allows for pre-tax contributions to a commuter account.
- Education Reimbursement for eligible education expenses. This policy is currently under review; additional information will be provided if any changes are made.
