Family and Medical Act (FMLA)
Personify Health maintains a policy that provides benefits for eligible employees who need time off from work for the birth, adoption or fostering of a child, for their own or a family member’s covered health condition, or for qualifying exigencies related to military service.
All employees of Personify Health who have been employed for at least one year and completed at least 1,250 hours of service during the 12-month period immediately preceding the commencement of leave, are eligible for up to 12 weeks of leave (or 26 weeks, as applicable) under the Family and Medical Leave Act (FMLA).

Note: In most cases, leaves will run concurrent with FMLA and any applicable state leave entitlements.
Types of Leave

Parental (birthing parent) | full time employees scheduled for at least 30 hours weekly after 90 day waiting period; must be taken within the first year of birth | 100% of weekly earnings up to 16 weeks: 6-8 weeks of Short Term Disability* plus 8 weeks bonding, offset by any applicable state family leave payments |
Parental (non-birthing parent) | full time employees scheduled for at least 30 hours weekly after 90 day waiting period; must be taken within the first year of birth | 100% of weekly earnings up to 8 weeks for bonding, offset by any applicable state family leave payments |
Parental (adoption/foster of child up to age 16) | full time employees scheduled for at least 30 hours weekly after 90 day waiting period; must be taken within the first year of placement | 100% of weekly earnings up to 8 weeks, offset by any applicable state family leave payments |
Short Term Disability (employee serious health condition) | full time employees scheduled for at least 30 hours weekly | 60% of weekly earnings not to exceed $3,400 per week for a period of up to 26 weeks* |
Long Term Disability | full time employees scheduled for at least 30 hours weekly | 60% of monthly earnings not to exceed $10,000 per month after Short Term Disability expires** |
Care for Family Member's Serious Health Condition | subject to FMLA eligibility provisions | unpaid |
